LifeMark Health is a privately owned and operated Canadian company. Since its inception
in November of 1998, the company has progressed quickly towards meeting its vision
and goal of becoming Canada's leading provider of integrated medical and rehabilitation
services. The company was built through an acquisition plan focused on bringing
together the top medical and rehabilitation providers across the country. Today,
LifeMark represents a consolidation of exceptional human resources, experience and
clinical expertise who work together to deliver the highest quality of health care
service to the Canadian population.
LifeMark Health, currently owns and operates 50 centres across Canada. These facilities
offer a variety of services and programs designed to meet the needs of their community.
Each LifeMark facility is staffed with clinicians who have the training and specific
interests to service their local area. All LifeMark staff strive to exceed the expectations
of all our clients through a unique relationship that leads to a positive outcome.

Commitment
LifeMark Health is committed to quality of care initiatives through the following
processes: accreditation, continuing education and outcome measurement. We strongly
support and encourage post-graduate, continuing education. Many LifeMark Health
clinics are accredited by The Canadian Council of Health Services Accreditation
(CCHSA), The Commission for the Accreditation of Rehabilitation Facilities (CARF)
and Institute of Work and Health (IWH).
Partners in Care:
The LifeMark facilities work closely with many different stakeholders through open
communication and an individualized approach to all assessment and treatment services.

clinics:
Individual Private Payors
A variety of medical and rehabilitation programs are provided to community members
by each LifeMark clinic. Many clinics have a strong sports medicine emphasis and
work closely with the injured athlete. LifeMark Health prides itself on providing
an interdisciplinary care model with key health care professionals as members of
the treatment team.
Employers
LifeMark Health acts as a “preferred” service provider with many employers. As a
national health services provider we are able to best address companies' needs in
all its locations across Canada. We have designated individuals who work together
with employers to deliver a full range of customized, results-oriented health management
services.
Insurers
LifeMark Health works closely with Life and Casualty Insurers across
Canada. We provide these companies with customized services to meet
their needs while at the same time providing cost effective and outcome oriented
results. Our team of clinicians regularly speak at seminars and conferences on a
variety of topics relating to health care.
Worker's Compensation Boards (WSIB/WCB)
LifeMark Health works with Worker's Compensation Boards in each province and is
committed to assisting injured workers in the return to work process. LifeMark
facilities across Canada are chosen providers by their respective
compensation boards to deliver a variety of programs and services to injured workers.
Provincial Government Funded Services
Designated LifeMark facilities offer health services that are publicly funded in
part or wholly by provincial governments.
Long Term Care and Retirement Homes
Canada's aging population represents a serious challenge with increasing demands
on the health care system especially in the area of rehabilitation. LifeMark is
committed to delivering premium quality physiotherapy service to an increasing number
of long term care and retirement homes in Ontario. LifeMark Health is a member of
the Ontario Long Term Care Association (OLTCA), the Ontario Non-Proft Homes and
Services For Seniors (OANHHS) and the Ontario Retirement Communities Association
(ORCA).
Hospitals
LifeMark Health partners with hospitals to manage on and off-site rehabilitation,
diagnostic imaging and other ambulatory services.
